WEST VIRGINIA
West Virginia residents may obtain a summary of the registration and financial documents from the Secretary of State, State Capitol, Charleston, West Virginia 25306. Registration does not imply endorsement.

VIRGINIA
Registered with the Commonwealth as required by law. A financial statement is available upon request from the State Office of Consumer Affairs at PO Box 1163, Richmond, Virginia 23218.

NEW YORK
Any solicitation of contributions by or on behalf of a charitable organization which is required to register and file annual financial reports with the Office of the Attorney General pursuant to Article 7-A of the Executive Law must include a statement that a copy of its latest annual report may be obtained upon request from the organization or from the Office of the Attorney General, Charities Bureau, 120 Broadway, New York, NY 10271.

FLORIDA
A copy of the official registration and financial information may be obtained from the Division of Consumer Services by calling toll-free 1.800.HELP.FLS within the state. Registration does not imply endorsement, approval, or recommendation by the state.

DELAWARE
No disclosure statement required.

KENTUCKY
No disclosure statement required.
